  • Our in-home care services are available 24/7 and range from daily living assistance to complex care.
  • Support workers deliver daily living assistance and personal care such as showering, grooming, domestic duties, house cleaning, and meal preparation.

FAQs

Home care services refers to supportive care of health care provided by registered, professional caregivers within the home where a client/patient is living, as opposed to care provided in group accommodations and nursing homes. Home care can cover anything from assisting with personal hygiene and care, dressing and undressing, transportation and mobility, rehabilitation schedules and more in hopes to allow loved ones to remain living independently in their own homes.

Depending on the level of your care needs, you may be eligible for home care services which can be accessed to you through the Commonwealth Home Support Programme or a “Home Care Package”.

You may be eligible if you’ve noticed a change in what you can do or remember, have been diagnosed with a medical condition or reduced mobility, experienced a change in family care arrangements, or experienced a recent fall or hospital admission. To qualify for an assessment, you need to assess how much assistance you require with certain everyday tasks, and if you are 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people).

There are some home care providers who deliver services to people with a disability younger than 65, including Bridges Alliance. You may be eligible to receive home care services through the NDIS. Eligible people must be:

  • Be aged under 65 years
  • Have a permanent disability that significantly affects their communication, mobility, self-care or self‑management.
  • An Australian resident *or equivalent

Depending on your home care package and funding, home care providers can provide the following:

  • personal care — such as help with showering and hygiene
  • communication support — help with using the phone or hearing aids
  • dietary support — assistance with nutrition and preparing meals
  • skin care and hygiene — such as bandages and dressings
  • continence care — including disposable pads, commode chairs and bedpans
  • mobility support — including crutches, handrails, wheelchairs and bed rails
  • support services — including leaning, gardening and transport
  • social support — help taking part in social activities and access emergency services
  • care management — reviewing agreements and making sure services are suitable
  • clinical care — including nursing, podiatry, physiotherapy and hearing services

Here at Bridges Alliance all our services are based on the current NDIS price guide and are applicable for all NDIS clients.

Outside of the NDIS, the cost of your home care services depends on your level of home care package. If you are eligible, the Australian Government may subsidise parts of your care to ensure that the help you receive is more affordable.

The total amount of your home care services consists of:

  • The Australian Government contribution (the subsidy).
  • Your contribution (the fees you may be asked to pay).

The funds together will cover your full care services as well as the package management costs from your home care provider to organise and manage your care services.

The breakdown of the government subsidy for each package level with an approximate yearly government contribution is as follows:

  • Level 1 – $9,000
  • Level 2 – $15,750
  • Level 3 – $34,250
  • Level 4 – $52,000

*Please note that you may have to wait 3–12+ months to be allocated your package.

How to apply for home care services?

With Bridges Alliance, you simply need to be a NDIS client and have the allocated funding for home care services – your support coordinator or local area coordinator can help you with this. That means if you’re an individual under 65 with a disability that significantly affects your communication, mobility, self-care or self‑management, you can easily request for home care services and regain lost independence in your own home with Bridges Alliance

As a registered NDIS provider, we can help you assess your eligibility with an online assessment and an easy phone call. From there, we’ll conduct a face-to-face assessment to discuss which home care services would be most beneficial to your situation, and hone in on which activities you want to be able to do. If you are approved, we can liaise with your support coordinator or local area coordinator on your behalf to develop a personalised support plan to help identify what sort of home care services suit your needs.
